Any individual doctor or hospital may participate as an eHealthManager provider if the doctor or hospital is qualified to render a covered service and complies with eHealthManager policies and procedures for provider participation. All providers of eHealthManager covered services must meet the following requirements:

Register with the eHealthManager which requires signing the Provider Agreement, Meet eHealthManager requirements for professional licensure, certification or registration.
